The woman, a Chabad emissary, I assume, has brought a lulav and etrog to the outdoor market in Jerusalem so that anyone who has not shaken them that day, as you’re supposed to every day during Sukkot, except on Shabbat, can do so. The boy is not wearing a yarmulke and since boys and men are supposed to cover their heads, especially while saying blessings, his mother is covering his head with her hand as he says the blessing before shaking the lulav and etrog.
